Introduction & Instructions 

This survey aims to assess current thromboprophylaxis practices for patients with cancer receiving ambulatory systemic therapies, including guideline adherence and gaps in best-practice implementation. We will also explore the role of pharmacists in improving cancer-associated thrombosis (CAT) prevention. 

 

Who should complete this survey? 

We are seeking responses at an institutional level, as the questions focus on institutional policies and standard practices. To ensure accuracy, please consult with relevant multidisciplinary colleagues at your healthcare service to ensure a representative response. 

 

Eligible respondents
✔ Pharmacy practitioners working in adult oncology or haematology treatment settings. 
✔ Those with knowledge or ability to access information on institutional practices and guidelines for the prevention of CAT. 

Note: Even if your institution does not have formal thromboprophylaxis guidelines, your input remains valuable in understanding real-world practices. 

 

Survey Details

  • Focus: Primarily on ambulatory care settings (outpatient/day admission), with some questions related to hospital inpatient care for context.
  • Estimated time: 15 minutes.
  • Confidentiality: Responses will be used for research purposes only and kept confidential to the study team. You will need to provide details about the healthcare service you are representing, to help identify and prevent duplicate responses. Your answers will not be publicly shared or published alongside your individual responses. Optional identifying information may be provided, for clarification or to participate in future collaboration or research and will remain confidential.
