We would like you to think about your experience as an inpatient at one of our hospitals. If you have had experiences in different areas of the hospital, you can complete the survey for each area. If you have trouble finding the area of the hospital you were in, please email survey@alfred.org.au.

The survey is anonymous and will not affect the care or treatment you are provided in any way. The survey will take around 10 minutes to complete.
